If you've gotten that far, then you're doing well. Unfortunately, you will need to create the checkboxes (and radio buttons, if you have them) yourself.
I assume you're using the LiveCycle Designer. If you will be using radio buttons, look up "Radio Button Group" in the online help in there and you'll see how to group them together so that only one will be on at a time. Of course, checkboxes don't need that treatment. (If you're not going to use radio buttons, sorry, but that tidbit would have saved you a lot of pain if you were. :) )
Draw one nice one, then copy and paste that as many times as you need it.
