How can I create an editable document from a PDF that contains both text and graphics?

I have a document that I would like to be able to edit and erase the text. The doc is pretty basic, in that it contains text and cloud bubbles with text inside. I attached the doc to show what I'm talking about.

I have access to Word, Excel, Acrobat but can only scan the doc into PDF format, so whatever method is used, it must originate from PDF format.
I've tried to convert the PDF to Doc, but the structure gets lost and I still can't edit the text from Word.

Any ideas?
