autopopulate html form

I am trying to autopopulate a form field . The form is located here http://movinglaborservices.net/optionstest/search/order.asp

When user clicks "click here" for origin provider they go to a search page to enter a zip code, once done it brings up the providers in the area. From there i want them to be able to click order this provider and it will auto populate the html form above with the providers ID #
