Suddently mail form stopped working!  Please Help!

NEED HELP Urgently!!!

I have an e-mail form that was working fine and suddenly quit working.  The resulting error is:

FrontPage Error.
User: please report details to this site's webmaster.
Webmaster: please see the server's application event log for more details.

Nothing shows in the event logs.  The server is Windows 2003, IIS6 and Frontpage 2003.  Peer to peer without domain or active directory.  I have tried the form as both an htm and an asp form.
I tried recalculating extensions and the entire site went down.  I had to restore the sites directory from backup to get the site back up and running.  I don't understand why the form stopped working in the first place as I had not made any changes to it.

Any help appreciated.  This matter is urgent!
