Front Page 2003 Forms do not forward to e-mail and do not present confirmation page when submit button is pressed

I have created asimple web page using Front Page 2003 which utilizes a few forms for visitors to submit information with.  The forms are simple text submissions.  When a visitor presses the "submit" button the page refreshes but does not display the Miscrosoft Front Page Default acknowledgement that the submission was successful and the form information does not get forwarded via e-mail.  The hosting company is winwebhosting.com and the site I am working on is located ay www.prayerpebble.com.

Attached is a test page I am using to try and clairfy and correct the issue, but it is reflective of all of the other form pages.
