just the other day forms on my web site started giving me an error instead of storing it in the forms handler and emailing it. the error is A username and password are being requested by http://www.southernparamed

does anyone know what file or setting this is refering to? thanks

- What changes did you make to the form?
- When did you add the other email addresses?
- Do you publish to http://www.example.com and not http://example.com?
- Did you try removing the CSV file and see if that helps (the permissions on the _private folder might be incorrect, especially if you are on a *NIX server)?
- Did you create another form with just a couple of inputs and one email address, publish that and see if that form works (this will help check to see if the form itself is incorrect)?
- While you are connected via FrontPage, you can go to Tools - Server and recalculate the extensions. Don't remove the extentions, just recalculate. Then you can go to Tools - Recalculate Hyperlinks and publish again.

This all started when I changed *change pages only to * all pages, overwriting pages already on destination.
I changed it back but no differince.
Can you provide the URL of the test form?
The recalculate hyperlinks is not the same. Does your hosting company provide a way to re-calculate the FPSE? Remember, don't remove them, just re-set them.
There really should not be a problem publishing all pages as far as FP - it just takes longer.
Since the form is not working - right click on the form in Frontpage and choose form properties. In there, remove the csv file and have only one email address. Don't edit the code, let FP do it. When you made those changes, let me know so I can check. I need to verify one thing as well on the form.

Does your mail server require SMTP authentication? Most usually do these days. And if so, contact your webhosting company to see if they can add the webserver's IP to bypass SMTP authentication.
