This all started when I changed *change pages only to * all pages, overwriting pages already on destination.
I changed it back but no differince.
Can you provide the URL of the test form?
The recalculate hyperlinks is not the same. Does your hosting company provide a way to re-calculate the FPSE? Remember, don't remove them, just re-set them.
There really should not be a problem publishing all pages as far as FP - it just takes longer.
Since the form is not working - right click on the form in Frontpage and choose form properties. In there, remove the csv file and have only one email address. Don't edit the code, let FP do it. When you made those changes, let me know so I can check. I need to verify one thing as well on the form.

Does your mail server require SMTP authentication? Most usually do these days. And if so, contact your webhosting company to see if they can add the webserver's IP to bypass SMTP authentication.
