UserID Check

I have a simple form that grabs the NTID. When the user submits the information, all of the records including the ID is sent to the table.

I would like to be able to check the NTID with the table so user will not enter multiple records (1 entry per user).

If the users ID exists in the table I would like to do a response.redirect to another page.

Any suggestions.
