I say use php from scratch as dreamweaver can only do about 90% and when it goes wrong you'll have to debug it.

Hmm... in a nutshell - use PHP to access the mysql database . and write it from scratch
see these tutorials for PHP. and code it.
however....
if you want to do it using dreamweavers built in cleverness then you have to setup the database (menu - window - tick databases) and set up the site, define the document type and set up the testing server.
once that is done you can then click on the + sign and define the mysql database connection then you can accesss the tables defined in the database and use dreamweavers cleverness to manipulate the records . and use the server behaviours to delete,insert addd records
