Software to analyze possible poisoning

I am looking for a software to analyze possible poisoning trends in a rural area where it is suspected that the water is contaminated with high levels of Arsenic . The software willl
collect
1. Patient Demographic data
2. Smoking / Nutritional habits etc.
3. Symptoms
4. Possibly photos of the patient

Does any anyone know of a software that can do this kind of thing?
