i have a web page that we use in powerschool  i can not get the health assessment box to hold a value. i have included the code and attached the web page

You need to have the input box inside of a form. See the example below, but you will also need the action to point to the page that will process the form.

If the same page is processing it, just leave action="" (blank).

what i don't understand is that the other text box's will hold the data this is the only one. i have attached the whole code.

Seeing the entire code, I'm not sure of the programming system running it, but it does seem that the name of the input field has to be very specific:

NAME="[01]Td4"

When you look through the code each have a naming scheme like that.

The form action goes here: /admin/changesrecorded.white.html, so you'll need to look through it and double check that the original programming that created the form doesn't actually require the field to be set up elsewhere before it's used in a form. And regardless, follow the scheme of that action file and set up the processing of your "Health" input field.
