Problem with Mediawiki saying cookies need to be enabled in order to log in

Trying to log in to a mediawiki v 1.13.1 on an Apache server, the user receives an error message that s/he needs to enable cookies in the browser.  The user is in fact logged in, but receives the error message nonetheless.

This happens on both firefox and ie.  We are attempting to set this up for school kids, and think that seeing this error message could stop them from going further on the site.

We already tried some instructions at mediawiki FAQ to change php settings, but it did not work.

This did not happen with the earlier version of mediawiki we were running.

Make sure in the setcookie commands the domain is set to your domain name for that site. It may be that the app isn't setting cookies due to this. Not sure though.

Double-check the version of each extension to make sure that it is the latest.
Also check the Discussion page for each extension to see if there are similar problems happening with particular extensions.

To be more exact with an answer, in addition to the version of MediaWiki, what versions of the following are you running:
apache
php
all mediawiki extensions you are using

Actually, it turns out that upgrading a single sign on extension for the wiki corrected the issue.
