I am about to do a study and as part of it, I want to create a web-survey with multiple questions (maybe 20-30).
Is there a website where I can create such a survey?
It should:
- Check IP so each user can only participate once
- maybe password for each user? So only the users that get a password from me can fill it out? Or something like that...

Depending on the kind of survey you want (small or big) or if you have a limited number of responders there's always:

but if you already have a website and sometime to invest I'd go with limesurvey:

It's also available in fantastico easy to install and configure (make sure you install in a separate folder though!!!
