I always use InstantRails from http://rubyforge.org/projects/instantrails/ as a base and update the ruby gems in there.
You may also want to look into the BidNami RubyStack at: http://bitnami.org/stack/rubystack

I find the easiest way to set up ruby on rails on vista is through Aptana RadRails. Its a full IDE and makes development easier.
