How to search db from asp?

Hi,

I have a search page that have 5 text boxes: First Name, Last Name, Gender, Specialty and City. I would like to be able to allow my users to search my database by either First Name, Last Name, Gender, Specialty or City. While I have written a simple search before but it's usually involve 1 text box, I have never written something like this before. I tried something like this for each of the 5 text boxes but that doesn't seem to work.
