Create user upload folders for social network type of site

I'm not sure how I should go about this.

I have figured out a way to use the 404 page search for a username to make a profile page... such as:
www.mywebsite.com/username  
and it will show their profile page, even though there is no real folder.

Now the question is, where should i store all the user's images?
If this site gets large should they be put one folder, or a system of folders based on their UserID field in MySql? Where can I store all the images from everyone? Should they all be in one gigantic folder? That doesn't seem smart to me. Are these folders real or are they just fake/masked by usernames?

I'm sure I'm not to clear with this, but it's hard for me to explain. I'm hoping someone might have an answer or a push in the right direction for me.
