Query data and image from MySQL database

Hi.

I have a MySQL database which holds information concerning my products.

- The table that holds all the info I need to query is called: ap_form_1

- The rows I'm trying to display are called: element_1, element_2, element_3 and element_4

- The row called element_4 stores only the names of the images; these images are actually located on my server like so: /data/form_1/files/ .

- The row called element_3 has the URL where I want the image that comes to link to

I'm trying to create a query to display the info of the products on an html page. I'd like for the image to link to its corresponding url (element_3)

I'm new to all this. What do I need to do? Bellow is what I have so far. The first three echo statements pull up the data, however I cannot get the image to display, nor get it to link to its corresponding url.

Thanks for your help! I'm stuck!
