Using PHP to change web page content at each access?

I've seen several applications where say the page accessed at http:///www.x.com shows one thing when you access it, but then if you do a page reload, the same page comes up with different content in the paragraph data, sometimes even different colors. The author said that he used ColdFusion to do that, but understood that the same thing could be accomplished with PHP. How?

For example, you have page header, banner, title, a few links, then in the center you have 2-3 paragraphs of text information. Upon page reload, the same page appears but the data in the 2-3 paragraphs is entirely different, including a couple of different links.

Does anyone know how this can be accomplished? And are there any special requirements and/or restrictions that the web hosting provider needs to have in order for this to work?
