PHP Form Generator from MYSQL Database

Is there such a thing as an application which will create an entry form based on a MYSQL table?

For example, I want you to give me the name of an application which I can give a MYSQL table to, and it will in turn automatically generate a form and database code for inserting new records into my database.

Any ideas?  Does such an application exist?  I found this auto form generator, but I don't want to type all my fields in, I want the app to figure the fields and field lenghts out using the MYSQL table.  Here's the app I found.

There are a few others frameworks that would be worth looking at:

* PEAR DB_DataObject combined with PEAR QuickForm http://pear.php.net/package/DB_DataObject/
* PRADO http://www.xisc.com/
* Propel http://propel.phpdb.org/trac/wiki/WikiStart
* phplens http://phplens.com/lens/

Of course, if you really want quick and dirty access to your online database, then you can't go past phpMyAdmin

You might give a try at dadabik http://www.dadabik.org/ which would be fine in most situations
