PHP Script to Submit form

I have a html form written with basic fields and a place to upload a document. I have a php script that will take the upload the document when the user clicks upload. I want to modify this script so that when the user clicks the submit button it uploads the document and saves the form off as a .txt or .html. file that can be reviewed by the appropriate department.

I do not know php. I have figured out just enough to edit this script to do what I need it to so far.
Both the form and script are attached. I had to change the extensions to .txt for it to upload. Thanks for your help.
