PHPBB3 getting hacked
This is 3rd time automatically this script has been added to my phpbb forum page. I have no clue how to deal with it. I have changed my password and in both ftp and database. But no result. Every time I had deleted the code from every have and re uploaded the files, but still its coming within a week. Do you have any solution of it?

have you updated to the latest version? 3.0.4
Also, did you delete the install directory when you installed the board?
Also check the permissions on the files
Should be the following
* Config.php: 666 before install, 644 after install
* Cache directory: 777
* Images/avatars directory: 777
* All other directories: 755
* All other files: 644
