Server and mailbox name check popup when opening Outlook connecting to Exchange 2013

Everytime our users open Outlook this box pops up asking to verify the Microsoft Exchange server name and mailbox name. The user's information is automatically filled in and clicking ok makes the box go away but it will eventually pop up again or when outlook is closed and re-opened.  When "cached mode" is disabled the problem goes away (but we would like to use cached mode).

The problem began when we migrated the old imap godaddy accounts to our new hosted exchange 2013.  It does not happen to all the accounts.
All Outlook profiles are newly configured via autodiscover.

Since it affects multiple users, it probably isn't anything related to the user or workstation. Although I would be tempted to delete a user's .OST file, just in case. After that, see if disabling the anti-virus and any local firewall on the system makes any difference. Following that, check the network setting and see if there are any issues at the firewall with traffic getting in/out to the Exchange host.

On this particular migration, It was only one PC. However it has happend before, and was the same sort of thing, only one or two PCs.  Its only when cached mode is selected so you my be on to something with the .ost.  You want me to delete the one created from the new profile correct?

Thanks for the help Brian, I will accept the solution as solved for now. I believe that will work, but have not had a chance to test it due to the user.  She was set up as using online mode to get rid of the problem and was fine with that.  Thanks alot man!
