I run a small business making web based database interface systems for small to mid-sized businesses. My business is primarily Intranet, not Internet.  My decisions are primarily focused on availability during the 9-5 hours, fast, and extremely stable.

My current design structure is to colocate very fast machines to run compiled read-only applications, which utilize SQL Azure for the database and Amazon S3 for document storage and serving static files via Amazon CloudFront.

My current interface structure is to utilize webservices to do all interactions with my database using AJAX calls from the client document.  This approach has proven extremely efficient.

My favorite toolset comes from http://dhtmlx.com  Their methodology is quite advanced, their support is excellent and their product is simply beyond compare.  After a decade in the business, I can say that these people are "tops" in their field.
