To collect data from a form, you need to use cgi, or email.  If
you don't have cgi access on your server, then I suggest you do
this.....make your form tag look like:
<FORM METHOD="POST" ACTION="mailto:your@email.address">

The message is going to be encoded by the cgi gateway, but you
will be able to figure it out as it is, and if you can't, there
are lots of freeware tools out there to decode them.  If you can
use cgi, let me know, and I'll help you write a script.
