How do I set up a server to ask for a user name and then a password before you enter a page.

This has almost nothing to do with CGI.  Restricting access to a page in a web server means configuring your web server.  With the http server, one can use the authentication module that is often built-in in the servers (Apache, etc.).  You can look into the documentation of your server to handle this since there are many ways to implement authentication:
1. using the system passwd
2. using DBM
3. using Berkley DBM
4. using mSQL

That is in the case of Apache but other NCSA-compatible should support as such.
