Put a database on your form, (It is found in the Data Access Tab)
Set LoginPrompt property to False,
Connect your database to an alias,
Give you database a DatabaseName,
You can give it any paramaters you need in the Params property
eg.
user name=PutUserNameHere
password=PutPasswordHere

You should be able to connect to the database.
