I would model my site after and try to improve on the idea.

There are several different routes that you can take that should meet your needs:
1.  Set up a feedback form in FrontPage 98.  This will give you a good starting point to get the information that you are looking for from the user.
2.  Then once you get this general form laid out you can choose to do several things with it either:
    A.  Have the Form Results E-Mail to you with the complaints.
    B.  Have the Form Results saved to a file on the server.
    C.  Use Active Server Pages (actually you could use several different things like CGI, Pearl, etc.) to save the Form Results into a database.

With Visual Interdev you should be able to set up the basics of this site within a week tops.  As far as hosting and running your own server, you'll have to check the prices out mine runs $60 a month $200 activation fee.
