Configuring Form Results

I am using the following form on a page at my site at http://www.geocities.com/Athens/Forum/3709/sbcs.html and have it configured to send the results of a submission to me automatically.

I want to know how I can configure the form to send results that would be set out as follows, so that I can simply cut and paste the text, without having to edit anything.

Sorry, but the simple answer is that you cannot change how the format of the email looks when it gets sent to you.  This is, as martike says, CGI driven and Geocities doesn't and won't give access to the the CGI-BIN directory for your own CGI script.  Sorry for the bad news, but that's just how Geocities and most other free web-site services operate.
