ASP and Forms

I am using ASP to create my connection between a form and a database (MS Access 97).  I am trying to create a form where the individual does not have to input the same general information into the form every time he/she visits.  It will be the only thing that never changes.
My form consists of text and combo boxes. Your help would be greatly appreciated.

If you do want to store all of this information on the server side (in the database) and not in cookies, then you will have to use ADO (via ASPs) to read the values from the database and let the ASP write client side script that will populate these combo boxes and text boses.

Could you give an example of the necessary steps in implementing this process.  Your help is greatly apprecitated. Also, if we need to, I can increase the points.
