You cannot use three radio buttons a a time.  The point of a radio button is to display a selction of a limited range.  Plainly put - radio button represent a question/input where there is only ONE answer.  By default, checking one automatically removes the aother.

If you want several answers, use 6 checkboxes.  This way, clicking one does not remove the checks in the other.  If you are set on using radio buttons, there is a way - but 5 points is a little low for that answer.  The checkbox methd should work fine. Good Luck!
