(1) You need to buy a C/C++ compiler (it doesn't come free with Windows).  Buy Microsoft or Borland, if you're starting as their are a lot more books to choose from.

Here are some recommendations (you can buy these online) :-

Microsoft Visual C++ Learning Edition to get started ($83 today) - go to http://www.software.net/AF11804/PKIN616314/prod.htm
This is really for NT4 though and you can't sell the programs you write (license restriction)

The standard version of Microsoft Visual C++ version 6 ($91.45) may be a better bet if you don't like the restrictions of the above.  The URL is
http://www.software.net/PKIN716859/prod.htm

Alternatively try Borland C++ Bulilder ($89 today - a discount) - go to
http://www.software.net/AF11804/PKIN665738/prod.htm
This is not quite standard C++ but is a lot of fun, and the easiest C++ way to write Windows programs

(2) Buy a good book on the introductory C++ language, browse your book store and find one you like - generally I'd recommend avoiding books like "Learn C++ with Microsoft Visual C++ version 5.0 for Windows 98" as these on the whole tend to date very quickly.  Also take a look at the samples that come with the compiler.
