Recommend Best Book/Web-Site on Online Form Creation in Word97?

I am in need of recommendations on the best books of web sites (besides this one) covering the following:

I need to make a number of online forms for a small (mom & pop-type) machine tool company. Among the types of forms needed and the things I need them to do are the following:

1. Generate shipping tickets.
  FEATURES NEEDED:
      a. auto-number ship-tickets
           - EXAMPLE: "1024", "1025", "1026", etc
      b. Series of dialog boxes that ask questions and automatically enter responses into ship-ticket for printing.
      c. Need form to ask how many copies of ticket   to print.....and then print that number.
      d. Need to be able to access a two-digit code  that would automatically launch a macro to fill out address fields.
      e. The ability to abort the fillin process.

2. Generate invoices.
  FEATURES NEEDED:
      Apart from those features mentioned in #1, this   form would also need to have an option for listing between 1 and 20 seperate items for billing.

I really appreciate any help you can give. I am pretty new to trying to work up something like this. It won't give me any extra money, but it will give me something to put on a resume for future use if I can pull this off. Thank you.

Get yourself a copy of Microsoft Access.
Go thru the tutorial that is provided.
You should then be able (with the help files/manuals provided) to be able to put together what you have described above.  Some of it you will probably be able to copy from the examples provided with Access (esecially Invoice templates etc).
