Go to the microsoft web site and look for the Help Developer's kit.  Your visual C disk does include this kit, but it's outdated and doesn't work properly.  It will certainly help you and there is full documentation (as far as Microsoft is concerned) on it.

There's also books available from www.amazon.com on Microsoft help that I found very useful.

Microsoft is changing their help scheme in VC6.0 and Win98 to go to HTML files.  You'll find the HTML Help developer's kit on the Microsoft web site too as well as a Microsoft blessed book at amazon.

I can't think of a reason that you can't use the Microsoft HTML Developer's kit with vc4.2, haven't done an incredible amount with it.

Good luck.
