create an email template in outlook

I'm trying to create an email template for use on an NT4.0 lan running MS Exchange, Office97, everyone using Outlook.  I'm trying to use the form design tool to alter the standard email template [renaming it as another template], but none of the form fields or labels stay with the email once it's sent.  If I call up the template, either to use it or edit it, it's all there.  But if I fill it in and email it to someone, an empty email arrives at the other end, stripped of all fields, labels and filled-in-data.  Help!

What does the other end means? Another server in a WAN environment? If so, you need to install the form onto that server also.

NO, the other end just means another user on the same lan.  I create a form on my workstation, then use it as a template to create a mail message that goes out over MS exchange to another users work station...but even in my own SENT box, the mail message is stripped of all its form data, all its form fields, and all its form labels.  All I see is the to,from, and suject lines, nothing else.

Did you publish the form into a public folder and open the form from the public folder?
If you didn't, you have to do so. Otherwise, the form you design will not be install on your machine.

I've tried a few forms with no problems.
