Settings dialog box for screensaver

How do you set up a settings dialog box like most screensavers have.  Say for example, I wanted to let the user select the number of falling leaves in my screensaver, how can this dialog be incorporated into the application>

Check out the General MFC Sample "SAVER: Screen Saver" that comes with Visual C++ and look into CSaverApp::DoConfig() in Saver.cpp.
