Form that sends e-mail & goes to a page on SUBMIT

I'm sure this can be easily done, but I'm kind of new to this and I can't figure it out.  I have a form which must be filled out and submitted in order for the user to download a file.  When they submit, I want the submitted info to be emailed to someone and the browser to advance to a page where they can click on a hyperlink to the "downloadable" file.  It seems very easy to me, but I can't get it to work.  I am using FrontPage98 and if I select email form results, it restricts me to e-mail only and the page stays on the form.

Thanks for the quick reply however I'm still stuck.  I tried the suggested code, but it opens a blank email message, it doesn't send the submitted form field data and it does not transfer the file.  I copied the code exactly as it is above and changed the email address to a valid one and the file name to the appropriate one.  One note - it is a selfextracting executable, not a zip if that makes a difference.
