First, search MSVC online help for the topic "Porting 16-Bit Code to 32-Bit Windows".  It contains a fair amount of information that will help you.

If you prefer online docs, check the following URLs:
http://premium.microsoft.com/msdn/library/devprods/vs6/vc++/vccore/_core_differences_between_16.2d.bit_windows_and_32.2d.bit_windows.3a_.overview.htm
http://premium.microsoft.com/msdn/library/devprods/vs6/vc++/vccore/_core_porting_16.2d.bit_code_to_32.2d.bit_windows.htm

Accessing the info requires registration to MSDN online but it is free and mostly painless.

That would be hard given that you haven't disclosed your address.  However, EE should send you notification messages as

Hmmm...  Did you read the URLs?  They *do* contain detailed instructions.  I don't think there is a better reference.

However, if you insist on a book, check this one:  "Porting to Win32: A Guide to Making Your Applications Ready for the 32-Bit Future of Windows" by Thomas Lauer.  ISBN: 0387945725
