building a private MFC42.dll

Anyone know how to build and link to a private build of MFC?  I found some info in TN033 on MSDN about how to build the dll, but can find nothing on how to use it once it is built.  Another viable solution would be for me to rename the dll, but I still need to know how to link to it.

"If you modify MFC in any way, you must rename the modified MFC DLL so it will not conflict with the MFC DLL that might be installed by other MFC applications on your user's system. This is not a recommended procedure."

There should be an import library .lib generated. Link with it.
