If this computer is Windows 95/98/98SE/NT then a registry edit may be in order.  I have worked with some modems that would not work until I removed every conceivable registry entry before the reinstall along with all references on the hard drive.  Most unsinstalls do not get all files or registry entries.  With the modem installed you can go to device manager and retrieve the driver file names to remove from your system.  Reinstall after that point and the problem should be resolved.

Start -> Settings -> Control Panel -> Modem -> COMx -> Diagnostics -> More info
and record the output from 'ATI3'.

Compare this value to the "text" within the specific '.INF' file which comes with the modem.
