snanning a form

How do I scan a form into Word?  I can save the document into Word but none of the formatting is correct.  All the text boxes are removed.  This is my first use with a scanner so I'm new at this.

johlari-the information was very helpful.  The web addresses were great and provided me valuable information.  For future reference, the Principia Remark Office OMR does not currently appear to support the HP ScanJet 3300C.  The Caere OmniForm 4.0 is exactly what I was after and does support my HP scanner.  I was also able to download an updated scanmanager.  Thank you
