Why can't any receivers receive my custom form sent to them?
i have published the form in my personal folders but still does not work.
Is there any mistake or any other ways to do it?

Have you checked your settings to allow RTF or HTML formatting on your messages.  If you are sending in plain text they will not see your form.  The mail clients that the receivers are using must also be setup to display the messages correctly.  Some clients will reject the forms or create an attachment of them.

Well....i have solved my problem...
it is because i did not edit my read page!!!!!!
