Identify modem

I am trying to identify my modem in an effort to set jumper switches to change its com port.  Its an ISA 56k modem with a rockwell chip.  The ATI6 gives this RCV56DPF L8570A rev 30.0/30.0.
There is no FCC number on it.

It is  AD56K-I (Retail Store K56flex based) Modems

If wanting to use with Win95/98, remove COM and IRQ jumpers.
Windows will assign those resources automatically.
