Banners with a selection box

In Dreamweaver, how do I create a banner ad with a selection box?  Do I use a table with layers?  I've tried to create one, but when I insert a selection list with a tag, it makes the banner bigger than the 468 x 60 constraint.   I know how to get an image in the banner, but I can't get the selection box and submit button to fit.

Try something like this:


<table border=0 background="yourbanner.gif" width="468" height="60">
      <tr>
      <td><select name="selection" size=3>
            <option> opt1
            <option> opt2
            <option> opt3
            </select>
      </td>
      <td><input type=submit value="Submit"></td>
    </tr>
</table>

Replace yourbanner.gif with the filename of your grafik

A good answer, but I know how to do the form.  I was looking for more specific details regarding the table, mulitple cells and how to place the form in a cell so it doesn't expand past the size limit.  I have figured it out though, so I don't need anymore help.
