Set all switches "up", i.e., away from the 'DIP ON' text which is under the switches.
Set switch 1 "down" to use IRQ7, or
set switch 2 "down" to use IRQ5, or
set switch 3 "down" to use IRQ4, or
set switch 4 "down" to use IRQ3, or
set switch 5 "down" to use IRQ2.

Leave switches 5 and 6 "up" to use COM4, or
set switch 5 "down" to use COM3, or
set switch 6 "down" to use COM2, or
set switches 5 and 6 "down" to use COM1.

Remember to disable the motherboard's COM1 (or COM2) port, if you set the modem to COM1 (or COM2).

Would you believe that I can answer this question because, earlier tonight, a friend brought her computer to my house for me to fix, and this computer contains a modem identical to yours, and the dip-switch settings are silk-screened onto the modem?
