This is not a function of sendmail, but rather your mail client. This is the simple mechanism for sending a binary file as the entire message:

  $ uuencode  mybinaryfile  mybinaryfile  >  mybinaryfile.uu
  $ mail user@host < mybinaryfile.uu

You can use this method to send compressed tar files to other Unix boxes or to PCs that have WinZIP.

If you want to know how the typical message attachments are handled by mail clients, you need to look into the multi-part MIME message standards. Basically, a message is broken into several pieces and each piece has a MIME header that tells its format. Binary sections are encoded in BASE64 format.

Or, in one step:
  uuencode mybinaryfile call-it-this | mail user@host
