What code do I have to add to/remove from an AppWizard-prepared MDI application in order for the program to start without a new document open?

In your CMyApp::InitInstance function add this code...

// Start up with no document open.
if (cmdInfo.m_nShellCommand == CCommandLineInfo::FileNew)
  cmdInfo.m_nShellCommand = CCommandLineInfo::FileNothing;
