I nned to ask you this:  Are you in charge of a 5200 server/cab and has no idea of the configuration ?
This is not something you play with if you dont know what you are doing.

Find the guy that configured it because it is impossible to know otherwise.

It is dependant on the configuration and yet there might be id-overrides, luxadm might be used etc.

Sorry but I not going get involved in something like this here.
Dont you guys have a support contract on this ?

Im sorry, if someone else like to tak this, feel free

I imagine that almost a month after this was opened that it's been fixed.

I'd use luxadm like besky suggested but another way of finding the disk is to find the World Wide Number of the failed disk...usually reported in the /var/adm/messages file. Then open up each enclosure. The WWN is printed right there on the disk. Then you know what enclosure it's in and whether it's front or rear slotted.

It's kinda a neanderthal way of doing it. But it does work.
