How do you send a custom form to an individual by email?

Publish the form to your personal forms library on your copy of outlook. Open the form, by going to Tools, Forms, Design a Form. Select the form you want, if it's already in your Personal Forms library then great. If not then once it's open, go to Tools, Forms, Publish Form As...
Make sure you change the Look In... box to Personal Forms library.
Once this is done, go to Tools, Options, Other, Advanced Options, Custom Forms, Manage Forms. Choose your form from the list on the right hand side, and click Save As...
You can then save the form as a form message (a fdm file). This can be attached to an email and then installed on someone else's PC.
