What is the string to include into a MS SQL script for a view that will pop-up with a prompt for user input (for example ZIP code, or a name)

There isn't one. You need a front end application to do that.

Something like a MS Access "Data Access Page"?

You can use access to connect to the database and create a front end in it but I would advise VB or ASP.
