take a look at the script i've linked there, it does what you want except states appear only for the usa when it's selected, it can be modified to your needs i think<br />

this is actually a javascript question in that case. if you mean by having states under the countries in terms of position, then you just need to add a &lt;br&gt; or two between the select boxes. the vaguely useful script is the one above, your best bet is to modify that, the problem i suppose is that not every country has states<br />
