Adding items to a list view

Hi, i want to know the code as to how i can add items to a list view from text boxes. For example i have a list view showing First Name, Last Name, And phone number. I want to add these data to the list view through text boxes. Please help.

You want to place TextBox on List View or you have three text box.
Let me clear
You want ,user enter data in a Text Box and after clicking on ok Button A form with List View will display with the data enter by the user.
or anything else
Please clear me
